    Team Captain Nomination Survey

    We are about to select our team captains for the upcoming season. Team leadership is vital for a season of significance. This is not a role to be taken lightly. The students selected for this position will be representing you so it's important to take this survey seriously. Below you will see several qualities, skills and characteristics of top notch team leaders. Each student is listed in alphabetical order on the overhead/flipchart/screen and has a number next to his/her name.

    Look at the numbers next to the names on the chart and then write up to four numbers next to each question/category. Make sure you take your time, think about your choices and accurately write the appropriate numbers.

    "Shows up" consistently with a positive approach for practice/competition.

    Has a true passion for the sport.

    Has a strong work ethic.

    Stays composed under pressure.

    Puts the team's goals ahead of personal goals.

    Willing to confront a difficult situation with teammates or players in a tactful fashion.

    Shows good sportsmanship. Wins with class, loses with dignity.

    Makes wise decisions on and off the field, court. (example, drugs/alcohol/tobacco)

    Respects coaching staff.

    Goes out of their way to "connect" with all teammates regardless of age or ability.

    Someone the team listens to and respects on and off the "field" of competition.

    If selected, this person would be committed to doing everything in their power to create a season that will be of significant-- regardless of performance or how many wins and losses we experience.
